Job Title: Senior Living Wound Care Nurse Practitioner (AGNP/CNP) | On-Site Rounding


Compensation: $120,000 to $160,000+ Total Compensation (Base Salary + Productivity Incentive Program) + Mileage Reimbursement

What You'll Be Doing

  • Provide advanced wound care services and community-based rounding throughout senior living communities.

  • Manage pressure injuries, diabetic wounds, post-surgical wounds, skin integrity concerns, and complex cases.

  • Support chronic disease management, preventative care, and care coordination efforts.

  • Collaborate with physicians, nurses, therapists, caregivers, and facility teams.

  • Help improve outcomes while reducing avoidable hospitalizations and complications.

  • Serve as a trusted clinical partner within the communities you support.


Qualifications

  • Active Minnesota Certified Nurse Practitioner (CNP) license (AGNP preferred).

  • Experience in wound care, geriatrics, primary care, post-acute care, skilled nursing, or related settings preferred.

  • Strong clinical judgment and confidence practicing independently.

  • Valid driver's license and ability to travel throughout the Twin Cities Metro Area.
