This full-time position is primarily remote, though it is deeply integrated within the framework of a Community Health Clinic. • Conduct comprehensive assessments and develop treatment plans tailored to individual client needs, including pre-crisis and crisis interventions.
• Maintain accurate documentation of client interactions and progress, complying with all regulatory and organizational standards.
• Employ evidence-based screening tools to assure effective evaluation and treatment planning aligned with clinical guidelines.
• Collaborate proactively with multidisciplinary teams to create positive behavior support plans that promote client wellness and recovery.
• Actively partake in licensing and accreditation processes, ensuring that all practices meet or exceed required standards.
• Serve as a liaison to various service systems, state agencies, and community partners, advocating for client needs and reinforcing program value.
• Foster open communication channels to enhance the delivery of care and adherence to agency standards.
• Engage with local, state, federal, and private organizations to broaden program outreach and secure potential partnerships and funding opportunities.

The clients you will support encompass a diverse array of backgrounds and mental health challenges. • A Master’s Degree in a relevant mental health discipline is required.
• An Independent Licensed Mental Health Counselor (LMHC) credential is essential for this role.
