Key Responsibilities
  • Serve as a primary liaison between clients, their families, and outpatient providers to coordinate care and communication effectively.
  • Engage with insurance companies to manage pre-certification processes and ensure proper utilization management for our clients.
  • Contribute to discharge planning and aftercare strategies, enhancing continuity of care for clients transitioning out of treatment.
  • Provide therapeutic meal support, utilizing evidence-based practices to model healthy relationships with food and nutrition.
  • Deliver safe, empathetic, and affirming care tailored to each client’s unique circumstances, recognizing and respecting their intersecting identities.
Patient Population / Client Focus

The ideal candidate should have a strong focus on serving diverse populations, including older adults and those facing a variety of mental health challenges. Our client base includes individuals dealing with trauma, depression, anxiety, and other complex mental health disorders. You will have the opportunity to apply your skills in various contexts, promoting wellness and providing healing across different age groups and backgrounds.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • A valid LCSW license in the state of Oregon is required.

Compensation and Benefits

The annual salary for this position ranges from USD 90,000 to 105,000 based on experience and qualifications.
