• Serve as a primary liaison between clients, their families, and outpatient providers to coordinate care and communication effectively.
• Engage with insurance companies to manage pre-certification processes and ensure proper utilization management for our clients.
• Contribute to discharge planning and aftercare strategies, enhancing continuity of care for clients transitioning out of treatment.
• Provide therapeutic meal support, utilizing evidence-based practices to model healthy relationships with food and nutrition.
• Deliver safe, empathetic, and affirming care tailored to each client’s unique circumstances, recognizing and respecting their intersecting identities.

The ideal candidate should have a strong focus on serving diverse populations, including older adults and those facing a variety of mental health challenges. Our client base includes individuals dealing with trauma, depression, anxiety, and other complex mental health disorders. • Master's degree in Social Work from an accredited institution.
• A valid LCSW license in the state of Oregon is required.
• Experience working with older adults and familiarity with their unique psychosocial needs is essential.
• Proven clinical and diagnostic skills, with the ability to assess and develop appropriate treatment plans based on individual client needs.
• Strong communication and interpersonal skills, with a focus on building positive relationships with clients and colleagues.
• A commitment to working both independently and collaboratively as part of a team, demonstrating leadership and initiative.

• Experience in a community health clinic or similar setting is preferred.
• Familiarity with evidence-based treatment methodologies and therapeutic approaches, particularly those relevant to older adults.
• Knowledge of insurance processes and regulations related to mental health care.

The annual salary for this position ranges from USD 90,000 to 105,000 based on experience and qualifications.
