Job description

PRINCIPAL MISSION: The Senior Manager is responsible for managing the Health Home Census as a whole, as well as the individual Case Managers, and promotes effective education, self-management support and timely healthcare delivery to achieve optimal quality and financial outcomes. The Senior Manager behaves in a professional manner, and consistently demonstrates and promotes the values of respect, honesty, care and dignity for the patient and all members of the healthcare team. The Senior Manager is committed to the constant pursuit of excellence in improving the health status of the community.

PRINCIPAL DUTIES AND RESPONSIBILITIES: Works in a manner that supports the mission and vision of SBH and performs in accordance with system-wide competencies and behaviors. Educates, and serves as a resource to providers on patients.

1. Promote a safe, cooperative and professional health care environment to ensure optimum patient care (as per HR Policy 113c).
2. Adheres to all hospital policies, procedures, rules and regulations, including but not limited to, absenteeism, ETIME, cellular telephone usage, dress code and rules of conduct.
3. Makes Customer Service a priority, treating customers (patients, visitors, and co-workers) in a professional manner, exercising courtesy and tact. Effectively communicates with patients and others about SBH services in their area(s) of expertise or responsibility and ensures that concerns, questions or issues reach the right person(s) in SBH in a timely fashion.
4. Demonstrates the standards of performance and behaviors consistent with the DRIVE to Patient-Centered Excellence.
5. Serves as a linkage between Health Home staff. SBH Behavioral Health and Medical providers and staff, patients and SBH management and administration to improve coordination of services and communications. Makes interdepartmental collaboration a priority.
6. Collaborates with SBH Behavioral Health in identifying appropriate patients for care management, utilizing established Care Management criteria. Supervises the referral process towards the goal of timely processing and enrollment.
7. Provides direction, support and supervision to employees. Holds periodic staff meetings to keep members current with procedural and policy issues, and to address any client issues and/or any staff issues
8. Monitors worklists for each employee to ensure timely documentation and procedural compliance with BAHN and NYS DOH Health Home regulations. Reviews and approves documentation when and where required. This is inclusive of consent, assessment (HARP and non-HARP), plan to care development (HARP and non-HARP), ongoing monthly contacts and referral management, and follow up on patient admissions or discharge from ER/hospital/residential/rehabilitative setting to facilitate the achievement of client goals and adherence to Health Home requirements.
9. Provides oversight and instruction to ensure coordination and linkage to behavioral health, medical and support services, as appropriate.
10. Acts as a conduit between Health Home, Behavioral and Medical providers and staff to ensure client needs are appropriately met. Provides crisis management coordination with providers and staff, as appropriate. Ensures timely completion and submission of all required incident documentation.
11. Responsible for training of new employees
12. Meets with patients, primary care and behavioral health providers, and other staff as needed to develop and coordinate treatment plans; meets with patient family members if needed.
13. Assumes an integral part of the Health Home Management Team and for the execution of initiatives developed by the Management Team.
14. Manages and enters E-time (Payroll), vacation and elective days prior to submitting to the Director. Enters information in Vacation Roster.
15. Manages the Healthfirst members' Gaps in Care, as relevant.
16. Reviews charts in BAHN and EPIC to ensure program compliance
17. Receives, reviews, processes referrals and creates MAPP Segments
18. Screens internal cases for Health Home Plus
19. Attends scheduled department and division meetings and participate as appropriate.
20. Other duties as assigned by Leadership.

Education Required:

  • Master's degree in social work or another human services field.

License(s) Required:

  • License not required
  • LMSW, LCSW, or LMHC preferred

Experience/ Skills

  • Two years management experience in a behavioral health setting
  • Three years in care management -with progressive responsibility
  • Ability to interact with all levels of staff in a professional manner
  • Proficiency in Microsoft Office, Excel, EMR
  • Strong written and verbal communication skills
  • Bilingual-Spanish/English (preferred)
