This position will be primarily onsite, requiring you to travel throughout the Ashburn area to provide home care services. Key Responsibilities
  • Direct and direct physical therapy treatment plans tailored to the individual needs of clients, aimed at preventing disability, relieving pain, and restoring muscle function.
  • Oversee and guide Physical Therapy Assistants to ensure effective execution of treatment plans.
  • Conduct comprehensive evaluations of clients' physical capabilities, utilizing advanced diagnostic tests to assess muscle, nerve, and joint functions.
  • Assess clients' home environments and make strategic recommendations for rehabilitation that enhance independence.
  • Deliver physical therapy services in clients' homes as authorized by prescribing physicians, ensuring adherence to their medical directives.
  • Administer therapeutic exercises, motivating clients in both manual and non-manual activities to facilitate recovery and mobility.
  • Evaluate and recommend modifications for prosthetic and orthotic devices in collaboration with medical professionals.
  • Actively participate in interdisciplinary case conferences to review and refine client care plans, ensuring holistic treatment approaches are employed.
  • Document all clinical assessments, treatment progress, and patient responses accurately for clinical records, including OASIS evaluations and discharge summaries.
  • Provide educational support to clients and their families on the effective use and maintenance of physical therapy devices.

Qualifications
