This position will be primarily onsite, requiring you to travel throughout the Ashburn area to provide home care services. You will work autonomously in clients' homes, developing and implementing therapy plans while collaborating with a multidisciplinary team. The support from administrative staff ensures that you can focus on delivering care without the burdens of excessive paperwork or logistics. • Direct and direct physical therapy treatment plans tailored to the individual needs of clients, aimed at preventing disability, relieving pain, and restoring muscle function.
• Oversee and guide Physical Therapy Assistants to ensure effective execution of treatment plans.
• Conduct comprehensive evaluations of clients' physical capabilities, utilizing advanced diagnostic tests to assess muscle, nerve, and joint functions.
• Assess clients' home environments and make strategic recommendations for rehabilitation that enhance independence.
• Deliver physical therapy services in clients' homes as authorized by prescribing physicians, ensuring adherence to their medical directives.
• Administer therapeutic exercises, motivating clients in both manual and non-manual activities to facilitate recovery and mobility.
• Evaluate and recommend modifications for prosthetic and orthotic devices in collaboration with medical professionals.
• Actively participate in interdisciplinary case conferences to review and refine client care plans, ensuring holistic treatment approaches are employed.
• Document all clinical assessments, treatment progress, and patient responses accurately for clinical records, including OASIS evaluations and discharge summaries.
• Provide educational support to clients and their families on the effective use and maintenance of physical therapy devices.

You will be working with a diverse patient population, primarily consisting of those requiring home-based rehabilitation, including elderly clients, post-operative patients, and individuals with chronic conditions. Your role will focus on empowering these clients to regain functional independence and improve their quality of life through thoughtful, personalized interventions.

• A Doctorate Degree in Physical Therapy (DPT) from an accredited institution.
• Current and valid physical therapy license in the state of Virginia.
• Certification in CPR and basic life support.
• A valid driver's license with automobile insurance is required for travel.
• Strong interpersonal and communication skills capable of engaging effectively with patients, families, and healthcare teams.
• A commitment to clinical excellence, integrity, and a patient-centered approach.

While a minimum of two years in physical therapy practice is required, preference will be given to candidates with home care experience, showcasing the ability to adapt skilled therapy in a community environment. The position offers a competitive salary range of USD 100,000 to 120,000 annually, commensurate with experience and qualifications.
