Job description

Community Support Services of Missouri (CSS) has been in operation since 1978 supporting children and adults with developmental, intellectual, and physical disabilities. Position Summary

This is a professional role working with individuals of all ages who have developmental disabilities. Case Management consists of connecting, planning and advocating for services for individuals served. As a member of a team, the Case Manager participates and confers with other mental health professionals in determining the proper treatment to purchase or procure for persons served. The Case Manager role requires a strong working knowledge and understanding of Case Management and on a limited basis will have other duties such as but not limited to; assisting the Management team, mentoring new and less experienced case managers, and participating in quality assurance activities. Supervision is received from the Director of Targeted Case Management.In addition to core case management duties, the role involves other duties, such as supporting the Management team, mentoring new and less experienced case managers, and participating in quality assurance activities.

Essential Functions

  • Interviews person served, their families, and others involved with the person served; assists in completing application for resources; collects basic data and obtains appropriate additional information from other agencies. Schedules and completes required assessments.
  • Develops, in cooperation with interdisciplinary teams, a Person-Centered Service Plan (PCSP) for each person served. Attends interdisciplinary team meetings to discuss persons served in relation to service plans, progress, and possible changes in the service program.
  • Completes PCSP monitoring per the DMH Directive.
  • Completes tasks and due dates in SetWorks with a 90% percentage rate.
  • Attends staff meetings and reviews new or revised policies and procedures.
  • Attends training as assigned by the supervisor. Completes training as required for new processes and directives from the Department of Mental Health.
  • Completes logging hours to meet or exceed the set annual average by the Director of Targeted Case Management and maintained within 3 working days of service.
  • Completes all service authorizations & budgets within the specified timelines.
  • Completes requests for new and increased services per Utilization Review regulations and timelines.
  • Keeps all information systems current; to include but not limited to CIMOR and SetWorks.
  • Represents the agency at community and/or Department of Mental Health (DMH) workgroups and committees as assigned.
  • Submits and attends DMH Due Process meeting to review modification of rights as needed.
  • Assist in the interview process, orientation and training of new employees as assigned.
  • Participates in QA and CARF outcomes as they occur.

Minimum Required Knowledge, Skills, and Abilities

  • Strong working knowledge of a specific discipline such as social work, psychology, special education, counseling, health care, or occupational therapy.
  • Strong working knowledge of case management methods, principles, and techniques.
  • Strong knowledge of various developmental disabilities and corresponding agencies and services available for clients.
  • Strong working knowledge of interviewing methods, principles, and techniques.
  • Ability to manage a caseload of clients with developmental disabilities, to keep service plans current, and to maintain accurate records.
  • Ability to collect and analyze information to make decisions concerning a person’s served support plan.
  • Ability to develop logical, realistic and practical service plans for persons with developmental disabilities.
  • Ability to negotiate and craft agreements with judgment and diplomacy.
  • Ability to lead and inspire colleagues to strive toward the agency’s vision and goals.
  • Ability to communicate effectively.

Required Education

  • A Bachelor’s degree from an accredited college or university or a registered nurse license.

Qualifications

  • Obtain and maintain all required trainings per DMH contract.
  • A valid driver’s license and good driving record, as well as proof of current auto insurance is required.

Physical Requirements

  • All employees should possess the ability to:
  • Lift 50 pounds or more on a repeated basis as required.
  • Potentially bend, stoop, kneel, and twist repeatedly.
  • Operate any required equipment with good dexterity.
  • Traverse changing terrain.
  • Periodically alternate between standing, sitting, and walking.

This is a full time position that will be primarily working in Cassville MO. This is a full time position wil access to benefits. We offer retirement matching of 3% and 100% vested day 1!
