Job Overview
As a member of the social work department, assist the PACE IDT in coordinating essential PACE services and promote participant satisfaction and retention through consistent, high-quality customer service skills. Lead efforts to support and address participant and family service questions, requests, complaints, appeals, and incident reporting. Ensure Medi-Cal and Medicare issues are effectively addressed via established workflows to promote prompt resolution. Responsibilities also include processing involuntary disenrollments, as well as facilitating on-site satisfaction surveys and actively engaging in other center-wide participant experience activities. Will assist participants and families with linkage and referrals to community resources and perform other case management activities as needed. Works collaboratively with MSWs and the interdisciplinary teams to achieve program and organizational goals while adhering to established state and federal regulations.
Minimum Requirements
  • Bachelor's degree in social work or another health or human services field required
  • A minimum of 2 years of customer service or case management experience is required, with a preference for experience in the healthcare field.
  • Knowledge of Medi-Cal, Medicare, and Social Security benefits is required, including application and renewal requirements, and related forms and documentation.
  • BLS/First Aid certificate is required.
  • Experience in PACE or other healthcare settings serving the frail or elderly is preferred.
  • Bilingual: Spanish/English, Chinese/English, or another second language is strongly preferred.

Compensation
$27.51 - $32.32 hourly
