Licensed Practical Nurse (LPN) - Canton, Maine

Job Summary:

Pinnacle Health & Rehab at Canton is seeking a dynamic and highly motivated Licensed Practical Nurse (LPN) to join our clinical team. As a Licensed Practical Nurse, you will be responsible for providing high-quality patient care in a 66-bed Assisted Living facility. We are proud of our consistent 5-star quality rating from the Centers for Medicare & Medicaid Services, which attests to our position as a top-performing facility in the region.

Responsibilities:

  • Provide direct patient care under the supervision of a Registered Nurse (RN) or other licensed healthcare providers

  • Lead and oversee a clinical team, working collaboratively with direct care workers and ancillary departments

  • Develop and implement individualized care plans for residents, ensuring a high level of customer satisfaction

  • Monitor and report changes in residents' conditions to the RN or other licensed healthcare providers

  • Participate in quality improvement initiatives and maintain accurate and up-to-date records

  • Collaborate with other healthcare professionals to ensure seamless continuity of care

Requirements:

  • Active Maine nursing license

  • Proof of MMR, varicella, Hep B, and seasonal flu vaccination (Oct-Apr)

  • Ability to work every other weekend with on-call responsibilities

  • Experience in Assisted Living Care is a plus, but not required. New graduates are encouraged to apply
