Job description

Work Skills is seeking a Lead Caregiver for our home care department in Howell, MI. This is a primary 7a-3p role with rotating weekends.
Lead Caregivers: Provide essential support to individuals facing social, cognitive, emotional, and physical challenges and assist the Community Services Supervisor. Additionally, the Lead Caregiver is responsible for providing guidance to caregivers, families, and persons served as needed.
Areas & Responsibilities:
  • Personal Care & Assistance
  • Instrumental Activities of Daily Living
  • Assists with maintaining client schedules.
  • Documents and reviews daily activities, observations, and any incidents according to the written plan of care
  • Mobility Assistance
  • Monitors the person-served for changes in condition and report changes promptly utilizing a proactive health and safety approach.
  • Identifies and effectively responds to emergency situations including behavioral, medical, and environmental per service plans and the EOP.
  • Identifies and reports potential health and safety concerns to ensure the safety and well-being of persons served.
  • Assists with emergency preparedness by completing regular emergency drills.
  • Assists the Community Services Supervisor with overseeing the medication program, specifically processing physician's scripts, submitting orders to pharmacies, entering new medications onto the medication program sheet, inventorying medications, and contacting pharmacies regarding orders.
  • Implements medication management upon receiving training on medication administration and compliance checks.
  • Accompanies individuals to medical appointments, maintaining accurate records of health-related activities and communicating any changes or treatment updates to the care team.
  • Emotional & Behavioral Support:
  • Provides opportunities for engagement in leisure activities and social interactions, encouraging participation in activities to build natural support.
  • Assists with welcoming new caregivers, conducting orientations, and supporting and/or conducting caregiver training in accordance with scheduled shifts.
  • Attends training sessions and meetings scheduled to stay updated on caregiving techniques and protocols per referral sources requirements and accreditation standards.
  • Commits to ongoing learning; encourages, supports, and mentors ongoing learning in persons served.

Education And Experience: High school diploma or GED equivalent preferred and may be required by a referral source. 1-year of caregiver experience is required for this role.

• Transportation: Have a valid driver's license with no restrictions as applicable and have reliable transportation to and from work. Drive and use one's own transportation while ensuring compliance with all legal requirements.
