Job description

Location: Sun Health Grandview Terrace - Sun City, AZ

Pay Transparency: $32.00 per hour for management duties (30 hours per week) + $40.00 per hour for personal training sessions, health coaching, and/or group fitness class delivery (up to 10 hours per week) + $7,500.00 Retention Bonus Opportunity (based on completion of 18-month pilot program); Minimum rates may vary based on local wage laws. Pay depends on factors including site location, specialty, certifications, work experience, and other business needs. 

Role and Responsibilities

The Activate Program Manager is responsible for managing all aspects of an 18-month Activate Pilot at Sun Health’s Grandview Terrace Senior Living Community. The Pilot will study how a progressive, data-driven, functional longevity program impacts the overall health and wellbeing of Sun Health residents. The successful completion of this Pilot will include: 1) Strong engagement among participants, 2) Low attrition, 3) Robust data collection, 4) Meaningful improvements in biometric health markers, functional independence, and overall well-being of participants. 

The Activate Program Manager is responsible for the administration of all aspects of the Activate Pilot. This person will provide oversight to maintain high quality standards and successful implementation of Activate programs and processes. The Activate Program Manager is responsible for creating an exceptional resident experience, thereby assisting with overall participant satisfaction and retention. 

The Activate Program Manager is responsible for carrying out Active’s commitment to service its members in the best environment possible with the friendliest staff, most educated members, cleanest facility, most ethical business practices and state of the art equipment. The Activate Program Manager will deliver on Active Wellness’ commitment to build healthy communities  through our core values of authenticity, teamwork, innovation, versatility, and excellence.

Specific Responsibilities

Essential Functions

  • Organize and implement Activate programs at Grandview Terrace.
  • Assist with development of marketing and promotions for the Activate Pilot.
  • Create health/wellness/fitness awareness and education among Activate Pilot participants.
  • Oversight and administration of onboarding process, assessments, data collection and storage, exercise prescription, nutrition guidance, and health coaching for participants in the Pilot.
  • Identify behavior and habit changes to help patients transform their lives.
  • Provide motivation, guidance, support, and encouragement.
  • Work one-on-one with clients to establish a personalized wellness plan to improve health metrics. 
  • Identify short-term and long-term exercise, nutrition, and weight management goals.
  • Assist clients in making behavior changes and providing on-going follow up.  
  • Track improvements in health behaviors and biometric measurements using provided technology platforms.
  • Proactively assist in the transition of program participants from Activate to general fitness activities at Grandview Terrace upon completion of the Pilot.
  • Meet key metrics for participation, adherence, and outcomes.
  • Assess and report results on a monthly, quarterly, and annual basis.
  • Develop and maintain supportive and productive relationships with residents, community leadership and staff, and health care providers.
  • Train, coach, support and manage Activate Pilot team members. Participate in the interview and hiring process (if applicable); submit requests for hiring (if applicable).
  • Manage participant questions, comments and concerns on a daily basis and provide mediation between members/staff, members/members and staff/staff.
  • Integrate standardized procedures and safety compliance within the Activate Pilot program.
  • Administer emergency care to members and staff in need of assistance.
  • Work as a TEAM with all other departments.
  • Other duties and responsibilities as assigned by supervisor.

Other Functions

  • Uphold Active Wellness written policies and procedures.
  • Uphold Grandview Terrace written policies and procedures.
  • Enforce policies fairly and consistently.
  • Assist in managing department Operational Standards of Excellence (OSE).
  • Conduct staff operational and safety meetings and trainings.
  • Manage Pilot program budget. 
  • Review, verify, and be accountable for department payroll submissions.
  • Submit purchase orders or expense reports for any and all departmental purchases according to established program guidelines.
  • Ensure all program staff members maintain appropriate certifications/licenses/permits as required by their role.
  • Be knowledgeable about all programs and activities in the Pilot. 
  • Assist with keeping the Pilot location well-stocked and clean.
  • Attend Active Wellness meetings and trainings virtually as required.
  • Handle injury/illness and security incident reporting.

Qualifications and Education Requirements

  • Preferred Associates or BS/BA in Exercise Science, Applied Health & Fitness, Kinesiology, Behavioral Science/Psychology, Nutrition and/or equivalent combination of education and experience.
  • Nationally recognized Personal Trainer certification.
  • Nationally recognized Health Coach certification.
  • Experience working with older adults in a fitness setting a plus.
  • Two years of program management experience in hospitality/fitness or related industry a plus.
  • Three to five years of experience delivering fitness, nutrition, and/or coaching programs to both individuals and groups in health and fitness settings.
  • Excellent communication skills, leadership and management skills, customer service oriented, results oriented, computer skills, ability to multi-task, experience in leading a team and commitment to fitness industry.
  • CPR/AED Certified.
  • Proof of citizenship or legal status.

Physical and Working Conditions

  • Ability to take the responsibility for the health and safety of others.
  • Ability to stand for several hours in a same shift; ability to lift 25 lbs.
