The Job

The Patient Care Coordinator will support SCAN clinicians and coordinate care for SCAN members to achieve SCAN's mission of keeping seniors healthy and independent.

You Will

  • Work closely with SCAN clinicians to coordinate and deliver excellent healthcare to our members.
  • Independently coordinate essential patient care by processing orders, relaying orders to vendors, arranging services, ensuring timely service delivery, and obtaining reports or results when applicable.
  • Coordinate patient appointments and transportation, ensuring specialists accept patient insurance and have all necessary documentation or authorization, visits are conducted on time, and that we obtain documentation and recommendations from visit.
  • Obtain patient care records and results from hospitals, doctor's offices, labs and other vendors.
  • Manage and triage incoming calls, mail, faxes and emails, including preparing forms for clinician completion and refilling approved prescriptions.
  • Maintain inventory of supplies and materials needed by clinical staff, including stocking and ordering supplies as necessary.
  • Conduct home visits with patients to complete assessments, deliver essential supplies, or facilitate virtual clinical visits.
  • Communicate professionally with members, caregivers, clinicians, vendors, and community partners while promoting the mission, vision, and values of SCAN Health Plan.
  • Seek out opportunities for improvements from trends in patient feedback or issues and propose viable solutions.

Your Qualifications

  • High School Diplomaor equivalent experience.
  • Associate's degree or equivalent experience preferred.
  • 1+ years of related experience required.
  • Training or certification as a Medical Assistant preferred
  • Performs work under general supervision.
  • Handles moderately complex issues and problems and refers more complex issues to higher-level staff.
  • Possesses solid working knowledge of subject matter.
  • May provide leadership, coaching, and/or mentoring to a subordinate group.
  • Bilingual Spanish proficiency is preferred but not required. Candidates without bilingual skills will be given full consideration based on their qualifications, experience, and ability to perform the essential functions of the role. (Test will be administered to assess proficiency if applicable.)
