ABOUT SAGEPOINT HEALTHCARE LLC
SagePoint Healthcare LLC is a licensed non-medical home care agency 
serving Medicaid Waiver and private pay clients in Springfield, Ohio. 
We are growing and seeking compassionate, dependable Personal Care 
Aides to provide in-home support services to elderly and disabled 
individuals. This role offers flexible scheduling, competitive pay, 
and the opportunity to make a real difference in someone's life.

GENERAL ACCOUNTABILITIES
- Assist clients with bathing, grooming, dressing, and hygiene
- Provide companionship, emotional support, and social engagement
- Perform light housekeeping, laundry, and meal preparation
- Transport clients to medical appointments and community activities
- Complete accurate EVV clock-in and clock-out for every visit
- Follow each client's individualized care plan
- Report any changes in client condition to the supervisor promptly
- Maintain client confidentiality in accordance with HIPAA
- Complete required documentation accurately and on time

JOB QUALIFICATIONS
- High school diploma or GED required
- Valid Ohio driver's license and reliable transportation
- Must pass BCI background check prior to start date
- STNA certification preferred; will train qualified candidates
- Previous caregiving or personal care experience a plus
- Compassionate, patient, and professional demeanor
- Ability to work flexible hours including evenings and weekends
- Must be punctual, dependable, and a team player
