Key Responsibilities
  • Conduct comprehensive assessments of clients to identify their mental, emotional, and social needs.
  • Develop personalized treatment plans based on individual client goals and therapeutic needs.
  • Implement evidence-based therapeutic interventions, utilizing cognitive-behavioral therapy (CBT) and other modalities to promote client recovery.
  • Maintain thorough and confidential documentation of client progress, treatment plans, and session notes in compliance with organizational policies.
  • Collaborate regularly with a multidisciplinary treatment team, contributing insights and feedback to enhance care strategies.
  • Engage in continuous professional development through attending workshops, participating in case consultations, and staying updated with the latest research in social work and mental health.
Patient Population / Client Focus

Our client population consists primarily of individuals grappling with mental health challenges such as trauma, anxiety, and depression, particularly those in hospice care contexts. As an LCSW at Empathic Sage Counseling, you will work with diverse clients, each with unique histories and cultural backgrounds, requiring sensitivity and adaptability in your approach.

Qualifications
  • A Masterโ€™s degree in Social Work from an accredited institution.
  • An active LCSW (Licensed Clinical Social Worker) license in the state of Colorado.

  • A commitment to working collaboratively within a team-oriented environment.
Preferred Experience
