Job description

The following job description identifies the qualifications and standards for Care Coordinators. Care Coordinators will meet the following requirements and competencies listed. This position description provides a general overview of the type of work performed by employees within this classification. It is not designed to contain or be interpreted as a comprehensive list of all duties, responsibilities, and qualifications that may be required of the employee assigned to the position. Depending on the facility, job duties may vary. Care Coordinators report directly to the Director of Care Services. I. II.

Job Summary:

The Care Coordinator serves an integral role by acting as a liaison between the pharmacy team and a partner facility. This is a great opportunity to utilize the skillset of a pharmacy technician in a non-traditional environment. Care Coordinators work outside of the pharmacy and onsite at the facility. Primary responsibilities include but are not limited to:

โ– Building strong relationships with clinic partners and consumers.

โ– Assisting with coordination of medication refills.

โ– Assisting with and resolving insurance barriers.

โ– Promoting the full services of Janus RX Pharmacy. A key function will be promoting Janus RX services to onboard and retain consumers and patients. A successful Care Coordinator will have strong communication skills and be able to perform detailed work.

Duties and Responsibilities:

โ– Build strong relationships with various Janus RX partners including consumers, case managers, and clinic staff to educate and deliver on the services Janus RX provides.

โ– Drive fun and exciting marketing events to promote Janus RX services in partnership with the clinic.

โ– Regularly enroll new consumers in our services and aid in continuity of care by coordinating prescription refills between providersโ€™ offices and subsequent delivery of medication to the patient.

โ– Work directly with providersโ€™ offices and/or facilities as part of an interdisciplinary team-based approach to medication management.

โ– Educate consumers on the ability to fill all additional prescriptions through the Janus RX Pharmacy in order to meet monthly goals of new consumer enrollment.

โ– Ensure all consumersโ€™ insurance information is up to date in the pharmacy system and is properly charged for the medications delivered. Maintain a high level of accuracy on consumer insurance information.

โ– Advocate for patient health by navigating common barriers such as prior authorizations, Patient Assistance Programs, sync fills, or delivery.

โ– Organize the collection of prescriptions to be fulfilled at the Janus RX Pharmacy and coordinate the delivery of medication.

โ– Execute monthly prescription refill calls to include compliance, monitoring medication refills, and insurance.

โ– Review and monitor regular reporting cycles and interpreting that data to drive actions with clinic staff and consumers.

โ– Administrative tasks including weekly reporting on medication discrepancies, dose changes, and discontinued medications for consumers.

โ– Inventory management as necessary by the facility partner.

Professional Responsibilities:

โ– Complete annual continuing education requirements.

โ– Maintain regulatory requirements.

โ– Represent the organization in a positive and professional manner.

โ– Comply with all organizational policies.

Regulatory Requirements:

โ– Possess and maintain a valid professional license/certification/registration issued by the State Board of Pharmacy.

โ– PTCB and / or NABP Certification

โ– Abide by and uphold the privacy rule standards of HIPAA.

Minimum Employment Requirements:

โ– High school diploma or GED.

Skills:

โ– Pharmacy Technician Certification Board (PTCB) certification.

โ– One (2) year experience as a pharmacy technician OR two (2) years of healthcare experience.

โ– Ability to read and communicate effectively in English.

โ– Moderate computer skills.

โ– Microsoft Excel

โ– Desire to help the underserved population in the mental and behavioral healthcare community.

โ– Strong desire to assist consumers with their medication plan to create the right outcome with an emphasis on customer care.
