Work Environment

This position is fully remote, allowing for flexibility while maintaining a strong connection to our clients and interdisciplinary team members. The hospice center environment focuses on providing dignified, compassionate care for individuals facing end-of-life challenges. Utilizing technology, you will engage with clients and their families effectively, ensuring that your support is always accessible.

Key Responsibilities

• Collaborate with families and outpatient providers to create comprehensive care plans that address emotional and social needs.
• Engage with insurance companies to manage pre-certifications and navigate utilization management processes, ensuring appropriate care is not financially hindered.
• Actively participate in discharge planning and aftercare coordination to facilitate client transitions while ensuring continuity of care.
• Provide nutritious therapeutic meal support, guiding clients in developing a healthy relationship with food.
• Deliver empathetic, culturally-sensitive care, tailoring support to address the unique identities and intersecting circumstances of each client.

Patient Population / Client Focus

In this role, you will primarily support older adults within our hospice center, addressing the complex emotional needs that arise during end-of-life transitions. Your focus will be on enhancing the quality of life for clients and their families, emphasizing communication, dignity, and shared decision-making.

Qualifications

• Master's Degree in Social Work from an accredited institution.
• Current and valid LCSW license in the state of New Jersey.
• Demonstrated experience working with older adults and understanding their unique challenges.
• Strong clinical assessment and diagnostic capabilities.
• Exceptional communication and interpersonal skills, fostering trusting relationships with clients and families.
• Adept at working both independently and collaboratively within a team environment.

Preferred Experience

• Previous experience in hospice or palliative care settings is highly desirable.
• Familiarity with electronic health record (EHR) systems and telehealth platforms.
• Training in trauma-informed care practices and cultural competency interventions.

Compensation and Benefits
