Job Summary:
  • Department: Physical Therapy
  • Shift Timings: 08:00 - 17:00 (Monday to Friday, 5 days/week, 8 hours/day)
    Weekly Hours: 40 hours
  • Location: Must be local to Livingston County, MI (primarily serving Brighton, Howell, Pinckney, Whitmore Lake, etc.)
  • Additional Details: Occasional weekend visits; no rotating shifts
  • Holiday Rotation: No holidays at this time
  • System Access: EPIC
Requirements:
  • Minimum 2 years of experience.
  • SNF experience required.
Responsibilities:
  • Conduct initial and ongoing assessments of patient conditions.
  • Develop, revise, and evaluate care plans in collaboration with patients and families.
  • Perform physical therapy interventions using standard techniques suited to the patient's condition.
  • Supervise physical therapy assistants, students, and support personnel.
