Locum NP Opportunity – Primary Care/Occ Med | Santa Clara, CA | $80/hr | M–W Schedule

Premise Health is seeking a skilled and compassionate Nurse Practitioner for a locum tenens role at their onsite clinic located at Roche Molecular in Santa Clara, CA. This is a premium W2 opportunity with a focus on employee health and wellness in a low-volume, supportive setting.


 Position Details:
  • Location: Roche Molecular, 2821 Scott Blvd., Santa Clara, CA 95050
  • Schedule: Monday–Wednesday, 8:00 AM–4:30 PM
  • Start Date: June 30, 2025
  • End Date: August 27, 2025 (with potential extension if no internal hire is made)
  • On-Call: None
  • Setting: Onsite employee health clinic

✅ Requirements:
  • Certifications:
    • Board Certified (ANCC or AANP)
    • DEA
    • BLS
  • Licensure: Active California NP license required
  • Experience:
    • Minimum 1 year of experience in Primary Care or Occupational Medicine

 Role Responsibilities:
  • Provide primary and occupational health services to employees
  • Manage acute and chronic conditions
  • Collaborate with clinic staff and support team
  • Use Epic EMR for documentation and communication

 Compensation & Perks:
  • Rate: $80/hour (W2, all-inclusive)

  • Orientation: 2–3 days
