We’re seeking a Registered Nurse (RN) to join our Assertive Community Treatment (ACT) team and provide community-based nursing care to adults with serious mental illness. In this role, you’ll work alongside behavioral health professionals to coordinate medical care, manage medication-related needs, provide health education, and help clients successfully manage their physical and behavioral health in the community.

Responsibilities

• Provide community-based nursing care, health assessments, screenings, follow-up care, and nursing treatment plans for ACT clients.
• Coordinate clients’ healthcare needs with physicians, pharmacies, primary care providers, behavioral health professionals, and other community providers.
• Monitor medications, side effects, laboratory needs, and overall physical health; provide medication administration and education as appropriate.
• Participate in interdisciplinary treatment planning, clinical assessments, case reviews, and transitions from hospitals or residential treatment back into the community.
• Maintain accurate and timely clinical documentation and participate in the ACT team’s on-call rotation.

Requirements

• Graduate of an accredited nursing program with a current Michigan Registered Nurse (RN) license.
• Minimum 2 years of professional nursing experience.
• Experience working with individuals receiving mental health or behavioral health services preferred.
• Working knowledge of physical health, chronic health conditions, medications, and coordination of care.
• Valid Michigan driver’s license and reliable transportation for community-based client visits.
• Ability to maintain required training, clinical competencies, confidentiality, and regulatory standards.
