Hospice Registered Nurse (RN) Case Manager Full-Time | Northbrook IL

Tired of high caseloads, frequent weekend rotations, and feeling stretched too thin?

At JourneyCare Hospice, we do things differently. We prioritize manageable caseloads, strong interdisciplinary support, and giving our nurses the time they need to truly care for patients and families.

We are seeking a compassionate and experienced Hospice RN Case Manager to provide patient-centered, end-of-life care in the Northbrook area.

Schedule & Territory
  • Territory: Northbrook, IL and surrounding communities.
  • MondayFriday, 8:30 AM 5:00 PM. No On-Call
  • One weekend rotation every 12 weeks and 1 summer and 1 winter holiday

Compensation & Benefits
  • $80,000 $97,000 annually (based on experience)

Key Responsibilities
  • Manage a dedicated hospice caseload as the primary RN Case Manager

  • Conduct routine visits and adjust care plans as needed
  • Provide end-of-life support, including family education and pronouncement

Qualifications
  • Graduate of an accredited RN program
  • Active Illinois RN license
  • Current CPR and First Aid certification
  • Hospice or home health experience preferred
  • Strong clinical judgment and communication skills
  • Valid drivers license and proof of insurance
