Registered Nurse (RN) – Home Health Baylor

3 Day Full-Time | Friday-Sunday or Saturday-Monday 

Preferred Care Home Health is seeking an experienced Home Health Registered Nurse (RN) to join our growing clinical team in a Full-Time Baylor position working your choice of Friday-Sunday or Saturday-Monday

This role is ideal for an experienced home health RN who is comfortable managing a patient caseload, completing comprehensive assessments, documenting in OASIS, and providing high-quality skilled nursing care in the home setting.

Position Responsibilities
  • Provide skilled nursing care to patients in their homes.
  • Manage an assigned home health patient caseload.
  • Perform comprehensive nursing assessments and evaluate patient needs.
  • Develop, implement, and update individualized plans of care.
  • Complete OASIS assessments and required clinical documentation accurately and timely.
  • Monitor patient condition and communicate changes in status to physicians and other members of the care team.
  • Administer medications and provide skilled nursing treatments as ordered.
  • Educate patients and caregivers regarding medications, treatments, disease processes, and home safety.
  • Coordinate care with physicians, therapists, aides, social workers, and other members of the interdisciplinary team.
  • Maintain compliance with Medicare, CMS, and home health regulatory requirements.
  • Promote quality outcomes and positive patient experiences.
Qualifications
  • Current and unrestricted Florida Registered Nurse (RN) license.
  • Minimum of 1 year of Medicare Home Health experience required.
  • Minimum of 1 year of OASIS documentation experience required.
  • Current CPR/BLS certification.
  • Valid driver's license, reliable transportation, and current auto insurance.
  • Strong clinical assessment, communication, organizational, and time-management skills.
  • Ability to work independently and effectively manage a home health caseload.
  • Knowledge of Medicare home health regulations and documentation requirements preferred.
