Location: Clewistown, FL
Job Type: Full-Time

MDS Coordinator - Skilled Nursing & Medicare Reimbursement
A skilled nursing facility in Clewistown, Florida is seeking an experienced MDS Coordinator to oversee resident assessments, Medicare reimbursement processes, PDPM accuracy, and clinical documentation.
This is not an entry-level MDS opportunity.
We are specifically looking for someone who has worked as an MDS Coordinator within a skilled nursing facility and understands how accurate assessments, documentation, coding, and timing directly impact Medicare reimbursement and facility revenue.

What We're Looking For
The ideal candidate understands both the clinical and reimbursement side of MDS.
You should be comfortable reviewing clinical documentation, identifying skilled services, managing assessment schedules, supporting accurate PDPM classification, and helping ensure the facility captures appropriate Medicare reimbursement based on the care and services provided.
Key Responsibilities
  • Complete and coordinate MDS assessments accurately and within required timelines
  • Manage the facility's MDS and RAI process
  • Oversee Medicare-related assessments and documentation
  • Monitor assessment schedules, ARDs, and required completion dates
  • Support accurate PDPM classification and Medicare reimbursement
  • Review nursing, therapy, physician, and interdisciplinary documentation for completeness and reimbursement support
  • Identify documentation gaps that could affect skilled coverage or reimbursement
  • Collaborate with nursing, therapy, rehabilitation, billing, and facility leadership
  • Participate in Medicare meetings and reimbursement reviews
  • Monitor skilled residents and Medicare coverage
  • Help ensure documentation supports the services being provided and billed
  • Review clinical indicators that impact reimbursement
  • Coordinate interdisciplinary care planning
  • Maintain compliance with federal, state, CMS, and facility requirements
  • Support audits, surveys, and documentation reviews
  • Educate clinical staff regarding documentation requirements and MDS-related processes
  • Help improve reimbursement accuracy while maintaining compliant clinical practices
Required Qualifications
  • Previous MDS Coordinator experience in a skilled nursing facility required
  • Strong knowledge of Medicare reimbursement in the SNF setting
  • Strong working knowledge of PDPM
  • Experience completing and coordinating MDS assessments
  • Knowledge of the RAI process and assessment scheduling
  • Experience reviewing clinical documentation for reimbursement support
  • Understanding of Medicare skilled coverage and documentation requirements
  • Strong attention to detail and organizational skills
  • Ability to work closely with nursing, therapy, billing, and administration
  • Strong communication and problem-solving skills
  • RN or LPN/LVN background preferred based on facility requirements
