We are seeking a Full-Time Home Health RN Clinical Manager. Below is a brief list of what we are seeking. Salary is commensurate with experience ($95K - $100K).

  • Five (5) years of Home Health experience (Medicare knowledge a must)
  • At least three (3) to five (5) years in a RN clinical management role
  • Working knowledge of SHP
  • Must be clinically sound and able provide training to new clinicians in the community.
  • Understands VBP and PDGM impact on home health services
  • Extensive OASIS knowledge
  • An understanding of Intake and Scheduling processes
  • Able to manage supplies during episodic care
  • Strong knowledge of wound care a plus
  • Willing to assist with Scheduling, Clinical Issues, and Patient Visits
  • Willing and able work ON-CALL at least once per month

System Knowledge – Must be system savvy (able to toggle between multiple screens and systems simultaneously)
