Position Purpose: Provides case management and supportive services for clients being treated in residential treatment. Facilitate client engagement in treatment services and successfully transition clients through levels of care to support early recovery. Provide continuity of program services in an ethical, legal, and moral manner within a safe and therapeutic environment, consistent with applicable regulatory and accreditation standards. Participate as a team member in the support of delivery of addictions treatment services to clients and their families and to facilitate the issues of recovery into their daily living situations.

Qualifications/Basic Job Requirements:
• Bachelor’s degree in Psychology, Counseling, Social Work, Addiction Studies or other related field, or equivalent experience and training
• Demonstrated education, training, or experience in the behavioral health/human services.
• Excellent written and oral communication skills,
• Computer proficiency required,
• Ability to navigate and complete documentation in the electronic healthcare record,
• Ability to accurately complete required documentation within the prescribed time frames
• Ability to quickly establish rapport with both clients and families; and motivate engagement in support systems identified through case management.
• Ability to present and coordinate lectures for clients, families, staff, or community resources.
• Self-starter with ability to work independently and within a team environment.
• Valid driver’s license and ability to meet Agency insurance requirements,
• Belief in the mission and vision of Rosecrance

Essential Responsibilities:

  • Work hours prescribed by the supervisor and any additional hours deemed necessary for the responsibilities assigned. Overtime may be required.
  • Work as a member of the residential treatment team.
    • Employ engagement strategies to increase client understanding of and motivation to complete treatment to support recovery.
    • Engage friends, family, and referral sources to support client engagement in treatment.
    • Facilitate case management sessions to support clients removing barriers to treatment and recovery.
    • Complete work-related documentation to support the early recovery process.
    • Assist with the completion of required documentation.
    • Facilitate external communication needs such as legal, employment, etc.
    • Collaboration with counseling, nursing, and psychiatric staff to support client’s clinical needs.
    • Transport clients as required.
  • Work as a member of the RHC treatment team.
    • Attend all scheduled meetings of staff, such as treatment plan reviews, shift meetings or appropriate committee meetings as designated.
    • Read all logs and obtain pertinent information from staff of previous shifts, counselors, staff, etc.
    • Record daily all pertinent information based on observations and involvement with the group and communicate this information to other staff.
    • Understand and comply with all principles established by the Rosecrance Corporate Compliance Program and Code of conduct.
    • Deliver exceptional customer service consistently to every customer.
    • Serve as a role model for other staff, clients and customers and demonstrate positive guest relations in representing Rosecrance.
    • Assume other related responsibilities as assigned by management

Schedule: 8-hour shifts

  • Monday - Friday; 9:00am - 5:30pm

Pay: Based on education, experience and credentials

  • Starting Pay:
    • Salary $46,800 equal to $22.50/hr.
    • Additional $.50/hr. with CADC certification

Shift differentials: Additional pay based on regularly scheduled shift hours

  • Second shift (majority of hours between 3pm and 11pm): $.25/hr.
  • Third shift (majority of hours between 10pm and 7am): $.50/hr.

Work Location: Rosecrance Harrison Campus - Rockford, IL
