Roadrunner Hospice is a compassionate, NM Nurse owned, patient-centered hospice agency dedicated to providing quality end-of-life care. We are committed to supporting our patients and their families through expert medical care, emotional support, and a focus on comfort and dignity.

Position Summary:
We are seeking an experienced and compassionateย Registered Nurse (RN) Case Manager(s) to join our hospice care team in the ABQ, and surroundingย areas. The RN Case Manager is responsible for the overall coordination of care for hospice patients, ensuring the delivery of high-quality care in accordance with the patientโ€™s individualized care plan.

Key Responsibilities:

  • Provide skilled nursing care and assessments to hospice patients in their homes or facility settings
  • Develop and implement individualized care plans in collaboration with the interdisciplinary team
  • Communicate regularly with physicians, patients, families, and the hospice care team to coordinate care
  • Educate patients and families on hospice philosophy, symptom management, and medication usage
  • Participate in on-call rotation to support patient needs after hours and weekends.
  • Maintain accurate and timely documentation in accordance with regulatory standards

Qualifications:

  • Current and unrestricted RN license.
  • Minimum of 1 year of nursing experience; hospice or palliative care experience preferred
  • Strong communication, organizational, and clinical assessment skills
  • Valid driverโ€™s license and reliable transportation
