Work Environment

This full-time position is uniquely remote, allowing you the flexibility to work from home while remaining an integral part of our supportive team. As part of our Hospice Center facility, you will engage with a diverse group of clients through telehealth platforms, which allows flexibility in scheduling, including evenings and some weekends.

Key Responsibilities
  • Conduct thorough psychosocial assessments that involve gathering detailed information about clients' mental health concerns, strengths, and available resources.
  • Deliver individual, family, and group therapy utilizing evidence-based treatment modalities tailored to the unique needs of each client.
  • Implement and supervise treatment plans, incorporating cognitive-behavioral therapy, solution-focused therapy, and trauma-informed interventions.
  • Collaborate effectively with a multidisciplinary team of psychiatrists, psychologists, and other professionals to ensure comprehensive and coordinated care for clients.
  • Facilitate group therapy sessions, psychoeducation classes, and support groups to foster community and shared learning among clients.
  • Provide crisis intervention, risk assessments, and safety planning as required, acting promptly and effectively to safeguard client welfare.
  • Maintain timely and accurate documentation in compliance with ethical and legal standards, ensuring client confidentiality and data integrity.
Patient Population / Client Focus

Qualifications
  • Master's degree in Social Work (MSW) from an accredited program.
