Heal and Revive Physical Therapy Center is seeking a dedicated Licensed Physical Therapist to join our team in Leesburg, Virginia. This position operates on an onsite basis as part of our dedicated home care facility. The work environment is dynamic, where you will interact directly with patients and their families. • Conduct comprehensive evaluations that involve screening and testing to assess physical function, identify limitations, and establish a diagnosis.
• Create individualized treatment plans tailored to each patient's specific needs, objectives, and preferences, using evidence-based approaches.
• Educate patients and their caregivers on therapeutic procedures, injury prevention, and strategies to enhance functional mobility both during and post-therapy.
• Demonstrate skillful application of various physical therapy techniques, utilizing modalities such as manual therapy, exercise biomechanics, and strength training.
• Maintain detailed and accurate documentation of patient progress and treatments in compliance with industry standards and regulatory requirements.
• Oversee and mentor physical therapy assistants and students, ensuring a consistent standard of high-quality care across all therapy sessions.
• Participate in quality improvement initiatives aimed at enhancing clinical outcomes and patient satisfaction rates.
• Assist in the management of departmental workflows, including scheduling, resources allocation, and equipment maintenance.

The Physical Therapist will cater to a diverse patient population, spanning all age groups from neonates to the geriatric community. Your role is pivotal in addressing a variety of conditions, including acute injuries, chronic pain disorders, and rehabilitation post-surgery. Emphasizing tailored approaches will involve understanding each patient’s specific cultural and social background, enhancing the effectiveness of care and fostering trust.

• A valid state Physical Therapist license is required.
• A Bachelor’s degree in Physical Therapy, with a preference for candidates holding a DPT (Doctor of Physical Therapy) or Master's degree from an accredited program.
• A minimum of two years of clinical experience in a healthcare setting, preferably with a home health focus.
• Strong communication skills and the ability to work collaboratively as part of a patient care team.
• Physical capabilities including carrying, lifting, manual dexterity, and standing for extended periods.

• Experience in home health settings or outpatient rehabilitation.
• Demonstrated ability to develop and implement innovative therapy programs.
• Knowledge of regulatory requirements and standards for home-based therapy services.
• Experience with electronic health records and patient management systems.

The salary for this full-time Physical Therapist position ranges from USD 100,000 to 120,000 annually, based on experience.
