Nurse Case Manager

We are seeking a highly organized and compassionate Nurse Case Manager to join our patient-centered care coordination team. In this impactful and fast-paced role, you will guide patients through their healthcare journey, ensuring they receive the right care, at the right time, in the most efficient and supportive way possible.

Your primary responsibilities will include assessing patient needs, developing individualized care plans, coordinating multidisciplinary services, and facilitating safe transitions across inpatient, outpatient, and community settings. You will review clinical information, monitor progress toward goals, advocate for patient needs, and work closely with physicians, therapists, social workers, and insurance providers to eliminate barriers to care. Additionally, you will educate patients and families about treatment plans, available resources, and follow-up requirements that promote long-term health and stability.

This role demands strong critical thinking, excellent communication, and the ability to navigate complex medical and social situations with professionalism and empathy. Requirements and Qualifications
