Summary of Position
The staff nurse provides direct nursing care to residents and delegates and supervises the care provided by nursing assistants. Takes ownership of assigned unit and acts accordingly to ensure effective operation of that unit. Administers medications, performs treatments and procedures, obtain specimens as needed and complete documentation as required.
Qualifications
  • Licensed Practical Nurse with a current State of TENNESSEE or Multi-State License.
  • Must be able to read regulations and patient records and write clearly.
  • Must be able to communicate in a courteous, friendly manner.
  • Ability to effectively manage and direct subordinates.
  • Physical requirements attached.

Education and/or Experience
  • Minimum LPN (Licensed Practical Nurse) Degree. Two years of experience working as a Licensed Practical Nurse including some medical/surgical experience and awareness of the needs of geriatric residents.

Primary Duties and Responsibilities
  • Provides daily supervision and leadership to assigned staff and directly provides nursing care to residents in Skilled Nursing Unit, Assisted Nursing Unit, Wellness Center, Dementia Unit, as assigned.
  • Walking rounds with off-going LPN and shift report to ensure proper communication has occurred between each shift.
  • Assess Residents every shift and be aware of any change in condition and act appropriately. Make rounds on every Resident at a minimum of every 2 hours.
  • Participates in the admission of new residents and ensure that all admission assessments are orders are entered and completion correctly.
  • Assist residents with Activities of Daily Living (ADL), family/social activities and actively promotes independent life-style of residents.
  • Responds to residents call system promptly and courteously and ensure all Resident's needs are taken care of in a timely manner.
  • Responsible for identification of resident medical and behavioral changes and the development and monitoring of care plans and re-assessment of each resident.
  • Provides daily report to the Director of Nurses, RN Supervisor, and/or Charge Nurse concerning facility conditions, personnel performance, resident conditions, and needs for nursing supplies.
  • Timely completion of work and task assigned within the scheduled time frame. Will not work overtime unless approved from RN Supervisor, Staffing Coordinator, or Director of Nursing.
  • Consistently demonstrates hospitality behavior and the core values of quality, respects, service and integrity.
  • Responsible for ensuring the needing weights are completed and entered in the system per policy.
  • Monitor meal service to ensure safety to all Residents.
  • Responsible to ensure all Residents are receiving the proper grooming, showers and oral care as assigned.
  • Provide hospitality to all Resident's, family members, visitors, and other staff members.
  • Work on the unit assigned by the RN Supervisor, Staffing Coordinator or Director of Nursing. May be subject to float to another unit as needed.
  • Provide complete Discharge instructions, education and medications to Resident at the time of discharge.
  • Timely completion of incident reports, audits and other reports as assigned.
