JOB TITLE: Community Support Professional โ€“ Respite

DEPARTMENT: Community Living

SUPERVISORโ€™S TITLE: Community Living Director

POSITIONS SUPERVISED: None

JOB STATUS: Full-Time, Non-Exempt

SCHEDULE: Varies; hours may vary to accommodate needs of person served.


Summary:


Provide community-based respite support to individuals with extraordinary support needs. This role focuses on enhancing quality of life by promoting independence, community participation, and meaningful daily experiences while ensuring health, safety, and person-centered care.


Key Responsibilities:


โ€ข Deliver respite services in alignment with each individualโ€™s Individualized Support Plan (ISP)

โ€ข Support individuals with daily living skills, personal care, and community integration

โ€ข Foster independence, self-advocacy, and informed decision-making

โ€ข Provide transportation to community activities, appointments, and services as needed

โ€ข Monitor individual progress and communicate updates to the team

โ€ข Complete accurate and timely documentation, including service notes and medication records

โ€ข Build and maintain positive relationships with individuals, families, and community partners

โ€ข Support individuals in leisure, recreational, and social activities

โ€ข Provide crisis intervention and emergency support when necessary
