Job description

Respiratory Care Navigator

The Respiratory Care Navigator (or RT Navigator) is a respiratory therapist who works in collaboration with the multidisciplinary care team to assess and coordinate chronic disease management for patients with chronic pulmonary diseases. The RT Navigator initiates and accepts patient referrals, assesses healthcare needs, identifies community and medical resources and manages assigned patient population. The RT Navigator works with case managers, coordinators and navigators across the system to assure standardized care and follow-up.

Minimum Qualifications:

  • Professional Technical Skills: Minimum 3 years direct patient care experience as an RRT required. Excellent assessment and documentation skills. Skilled at navigating multiple EMRs (NextGen, Meditech) and Microsoft Office applications. Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. Good written and oral communication skills.
  • Licenses or Certifications: Indiana State Licensure and RRT by the NBRC Annual CPR certification (BLS) Credentialed as a COPD Educator via the American Association for Respiratory Care (AARC) course within 1 year of hire.
  • Education: High School Diploma or GED CRT, or RRT by the NBRC Graduate of a CoARC accredited program in Respiratory Therapy, Associate’s degree required. Bachelor’s degree in associated allied healthcare field preferred.
  • Other Skills and Requirements: High level of professionalism with the ability to maintain confidentiality Strong attention to detail and accuracy, excellent organizational skills with ability to prioritize, coordinate and simultaneously maintain multiple projects with high level quality and productivity. Customer Service oriented personality with ability to work well under pressure. Skilled to care for certain age-related patient groups: all age groups.
