Job Title: Resource Navigator - BilingualStatus: Non-Exempt Reports To: Senior Services Supervisor Supervises: N/A Location: Access to Healthcare Network – Reno Office About Access to Healthcare Network (AHN)Access to Healthcare Network (AHN) is a mission‑driven nonprofit dedicated to improving access to quality healthcare and supportive services. Joining AHN means contributing to meaningful work that positively impacts seniors, individuals with disabilities, and underserved community members.JOB PURPOSEThe Resource Navigator position provides person-centered counseling and assistance to seniors, individuals with disabilities, caregivers, and community members. This role involves helping consumers identify needs, understand available options, access community resources, complete applications, and connect with appropriate programs and services. This position requires extensive travel within the Northern Nevada counties.KEY RESPONSIBILITIESInformation & Resource NavigationAssess consumer needs and goals and collaborate with consumers to develop person-centered service plans.Provide information, education, and guidance on available community resources, programs, and long-term services and support to help consumers make informed decisions.Evaluate preliminary eligibility for public and private benefit programs and assist with applications and enrollment processes.Advocacy & PartnershipsIdentify barriers and advocate on behalf of consumers and their families with service providers, agencies and community organizations.Maintain collaborative relationships with public and private community partners to support effective service delivery and coordination.Documentation & ComplianceDocument all client contacts, services, referrals, and outcomes in required databases.Ensure documentation is timely, accurate, and compliant with program standards and HIPAA requirements.Outreach & Service DeliveryProvide services by phone, email, or in-person at approved locations throughout the service area.Participate in outreach, marketing, and public education activities, including community events, presentations, and resource fairs.Professional ResponsibilitiesMaintain working knowledge of AHN programs, systems, and required databases.Communicate effectively with supervisors and internal departments.Attend staff meetings and supervision sessions.Adapt to changing priorities and perform other duties as assigned.EDUCATION & EXPERIENCE REQUIREDBilingual (Spanish/English) required.High School Diploma required; Bachelor's degree in social work, public health, psychology, or a related field preferred.Must obtain Medicare Assistance Program Level II Certification within 60 days of hire.Minimum of 2 years of experience in social services, care coordination, or related field.Proficiency in Microsoft Office and database systems.Experience working with seniors, people with disabilities, and/or diverse populations preferred.Flexibility to accommodate department-determined work schedules, which may change based on operational needs.Valid driver's license and ability to operate and AHN company vehicle as needed.Must adhere to HIPAA regulations.WORK ENVIRONMENTThe noise level in the work environment is usually moderate, and the employee usually works in a climate-controlled office environment.Ability to travel and drive for extended periods (up to four hours at a time).Be able to sit at a computer for 6–8 hours per day.Be able to stand between 3–4 hours per day.Be able to withstand hand/wrist deviation and repetition.Must be able to lift, carry up to 30 pounds with a fair consistency.The statements herein are intended to describe the general nature and level of the position, but are not necessarily a complete list of responsibilities, duties and skills required of employee(s) so classified.
