Position Overview:

The primary role of the Senior Living Advisor for Health Care and Assisted Living is to serve as a member of an integrated team facilitating admissions to the Bluestem life plan community of The Cedars through collaboration with internal and external stakeholders. The Senior Living Advisor will establish and maintain strong community relations with staff and members of the medical community to ensure coordination of services.

Essential Job Functions:

Community Relations

  • Promote Bluestem Health Care and Assisted Living residency in coordination with the Bluestem-wide Senior Living Advisor team:
    • Deliver Marketing Materials to discharge planners, physician offices and other community entities.
    • Assist in planning, coordinating and attending events in the community to promote services provided at Bluestem Communities and support assisted living, health care, and rehab visibility and occupancy.
    • This may include work outside of regular hours at various marketing events, potentially including some weekends.
  • Build and nurture relationships for referral development including but not limited to physicians, hospitals, discharge planners, case managers, and rehabilitations hospitals in McPherson, Hutchinson, Salina and surrounding areas.
  • Foster good working relationship with hospital case managers.
  • Collaborate with the Bluestem marketing team as needed to support visibility and awareness of healthcare and assisted living services.
    • Marketing functions will follow the Bluestem Communities shared services structure.
  • Participate in cross-campus events when appropriate to support transitions and collaboration.
  • Carry out other general sales tasks that may arise in the organization or assigned by Vice President of Marketing & Sales.

Manage inquiries related to new, incoming residents – Pre-Admission Responsibilities:

  • Receive and respond promptly to inquiries and provide information to prospects.
  • Schedule and facilitate tours for Health Care and Assisted Living prospects.
  • Consistently utilize the Customer Relationship Management (CRM) system to track all prospect interactions and ensure adherence to the defined sales process.
    •  Maintain accurate and up-to-date records in the CRM.
  • Respond promptly to inquiries for resident placement and gather comprehensive overview of medical and financial history and share with internal stakeholders, including:
    • Initially determine whether needs can be met by Bluestem communities.
    • Provide detailed prospect information, including physical and mental status, to admissions team.  
    • Communicate with the key admissions stakeholders’ team at designated Bluestem Community campuses to review and verify placement appropriateness.
    • Provide all related detail to decision makers, participate in review to ensure proper placement.
    • Provide business office with insurance verification and insurance pre-authorization information.

Post-Admission Responsibilities:

  • Facilitate smooth transition from hospital to skilled or long-term care setting including discharge notes, physician orders, CARE assessment, transportation arrangements.

Support / follow health episodes of current residents

  • Obtain hospital updates on current residents and coordinate return to Bluestem residence including arranging transportation and re-admission orders.
  • Provide subject matter expertise / collaborate and support to Director of Independent living during resident emergent health concerns and connecting to appropriate resources.
    • When transitions occur support with details about higher level of care options – Assisted Living or Health Care.

Qualifications and Experience:

  • Must be at least 18-years of age
  • Prefer (3) three-years' experience in case management, care coordination and discharge planning within a health care setting
  • Must have valid KS driver’s license and meet Bluestem driving requirements
  • Must be able to provide own transportation to appointments and hospital visits
  • Must become familiar with and be able utilize software programs relevant to the position
  • Proven quality customer service and communication skills with ability to overcome objections

Physical Requirements:

Light Work – Exerting up to 20 pounds of force occasionally, and/or up to 10 pounds of force frequently. Requires walking or standing to a significant degree; or requires sitting most of the time but entails pushing and/or pulling of arm or leg controls.
