Job Summary
The Resident Care Coordinator (RCC) is responsible for coordinating resident care needs and staffing, including maintaining the schedule, updating coordination tools like the physician appointment and pharmacy logs, filing and organization of the clinical records, etc. The Resident Care Coordinator assists the Director of Health Services in training the new care staff. When the Director of Health Services is absent, the Resident Care Coordinator is responsible for the supervision and delivery of resident care services in compliance with Ovation's policies and state and federal regulations.


Responsibilities:

  • Document information pertaining to changes in residents’ status, needs, and appointments.
  • Ensure proper documentation of all care plans, nurses' notes, and all other resident correspondence.
  • Provide orientation and on-the-job training to Caregivers and Medication Techs.
  • Assist with daily living activities (i.e., bathing, dressing, and eating).
  • Conduct weekly, monthly, and quarterly audits.
  • Complete quarterly evaluations of residents as assigned by the Director of Health Services and/or Executive Director.
  • Participate and provide feedback in Quality Assurance Meetings.
  • Manage scheduling of Caregiver and Medication Tech staff, ensuring adequate staff are on duty at all times.
  • Participate in morning stand-up meetings, providing recommendations and feedback as appropriate.
  • Create and maintain an atmosphere of warmth, patience, enthusiasm, calm, and joy.
  • Complete ongoing training as assigned.
  • Support caregivers in daily resident care, communication, and problem-solving.
  • Assist with new resident assessments, move-ins, and transitions.
  • Ensure that adequate supplies and equipment are on hand to meet the day-to-day operational needs of the facility and residents.
  • Participate in training and support staff development efforts.
  • Maintain a clean and organized environment.
  • Follow established healthcare standards and practices.
  • Ensure the safety and comfort of residents.
  • Communicate effectively with residents, family members, and staff.
  • On-call staffing responsibilities after hours and weekends as needed.
  • Other duties assigned are appropriate to the position.

Requirements

  • Prefer experience as a Caregiver and Medication Tech in a Memory Care/ Unit or Assisted Living Facility.
  • Must maintain and have an active CPR/First Aid Certification during employment.
  • Must have the ability to obtain a Food Handler’s Card.
  • Experience in electronic medical records documentation.
  • Must have a high school diploma or equivalent.
  • Prefer a licensed Certified Nursing Assistant in the State position the position is held.
  • Must be 18 years or older.
  • Friendly, attentive, and professional demeanor with residents and guests.
  • Maintain confidentiality of all resident care information.
  • Compassion for and desire to work with the elderly
  • Create and uphold an atmosphere of warmth, patience, enthusiasm, and a calm and cheerful environment.
