Position Overview

Alvita Counseling Center is seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our esteemed hospice team on a full-time basis. Work Environment

Our work setting is predominantly remote, allowing our social workers to provide care from the comfort of their own homes while remaining actively involved in the lives of our patients and their families. Despite the remote nature of this role, you will be part of a closely-knit interdisciplinary team of healthcare professionals, including nurses, physicians, and other specialists who collaborate to create a holistic care environment focused on patient-centered outcomes.

Key Responsibilities
  • Conduct in-depth psychosocial assessments through patient and family interviews, observations, and a review of medical histories to identify emotional, social, and environmental factors affecting the recovery and comfort of patients.
  • Deliver counseling services and provide support to patients and their families, assisting them in coping with end-of-life issues, adjustment to terminal diagnoses, and grief.
  • Work closely with the rehabilitation team to formulate and implement individualized treatment plans that address the psychosocial needs of each patient.
  • Facilitate access to community resources, including support groups, financial assistance, and palliative care services, ensuring patients and families receive additional support as needed.
  • Offer education and resources to patients and their families regarding post-rehabilitation services, thereby empowering them to make informed decisions about their care and support options.
  • Participate in discharge planning, ensuring safe transitions for patients moving to home care or other facilities are handled appropriately.
Patient Population / Client Focus

The patients you will support at our hospice center are predominantly individuals facing terminal illnesses, including but not limited to cancer, advanced heart disease, and neurodegenerative disorders. You will assist not only the patients but also their families, who are often navigating complex emotional landscapes filled with uncertainty, grief, and frustration.

Qualifications
  • A Master's degree in Social Work from an accredited institution is required for this role.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) is mandatory.
  • A strong understanding of crisis intervention, grief counseling, and family dynamics, particularly in the context of end-of-life care.
  • Excellent verbal and written communication skills, as well as strong interpersonal skills to connect with patients and families effectively.
Preferred Experience
  • Previous experience in a hospice, palliative care, or rehabilitation environment is preferred, as it equips candidates with the necessary understanding of the clinical challenges faced in these settings.
  • Familiarity with community resources in Kansas that provide additional support for families dealing with terminal illnesses.
