• Conduct thorough mental health assessments, including psychosocial evaluations, diagnostic interviews, and risk assessments, tailored to the unique needs of hospice patients.
• Collaborate with patients and their families to develop individualized treatment plans that address specific psychological, emotional, and cultural considerations.
• Utilize evidence-based therapeutic interventions, such as cognitive-behavioral therapy (CBT) and solution-focused brief therapy (SFBT), in both individual sessions and family group settings.
• Provide crisis intervention services as needed, working closely with an interdisciplinary team to ensure continuity of care for patients experiencing acute distress.
• Maintain meticulous clinical documentation in accordance with best practices and regulatory requirements, ensuring patient confidentiality and adherence to ethical standards.
• Offer psychoeducation to patients and families, equipping them with knowledge to manage anxiety, depression, and other mental health conditions prevalent in hospice care.
• Engage in regular outcome assessments to evaluate the effectiveness of treatment interventions and make necessary adjustments to care plans.
• Stay current with the latest research and emerging trends in mental health, integrating this knowledge into clinical practice for the benefit of your patients.
• Work closely with a multidisciplinary team to coordinate comprehensive care that prioritizes the emotional, psychological, and social needs of patients.
• Ensure effective communication with healthcare colleagues, creating a seamless support system for patients and their families.

This role primarily involves working with patients facing terminal illnesses in a hospice environment. You will provide critical emotional and psychological support to individuals and families during one of the most challenging times in their lives. Your expertise will help them navigate grief, loss, and the complexities of facing life-limiting conditions, ensuring they have access to compassionate care that respects their perspectives and wishes.

• Master's degree in Social Work from an accredited program.
• Current and active state licensure as a Licensed Clinical Social Worker (LCSW).
• Proven ability to work independently while also being an effective member of a collaborative healthcare team.
• Exceptional interpersonal skills with the ability to build rapport and trust with individuals from various cultural backgrounds.
• Strong written and verbal communication skills for effective documentation and interaction with patients and healthcare colleagues.

• A minimum of 3 years of clinical experience in social work, particularly in hospice, palliative care, or related settings.
• Experience with evidence-based therapeutic modalities and practices in individual and group therapy.
• Familiarity with community resources available to patients and families in hospice care.
• Proficiency in teletherapy technologies and experience conducting remote sessions.

• The annual base salary for this position ranges from USD 115,000 to 130,000, depending on experience and qualifications.
