Key Responsibilities

As a clinical social worker at Revitalize Mental Psychotherapy Center, you will:

• Conduct thorough psychosocial assessments to identify emotional, social, and financial barriers impacting patient care.
• Collaborate actively with medical teams to create and execute integrated treatment plans that address both psychological and physical health needs.
• Provide individual and group counseling to patients facing various issues such as trauma, grief, and coping with chronic illness.
• Facilitate connections with community resources to support patient discharge plans, ensuring seamless continuity of care post-hospitalization.
• Educate patients and their families on available treatment options and community resources, empowering them to make informed decisions.
• Participate in hospital committees, lending your expertise on social work matters to enhance overall patient care practices.
• Provide crisis intervention as needed, offering emotional support during critical situations.

Qualifications

Compensation and Benefits
