Key Responsibilities

• Conduct detailed mental health assessments of patients and their families, carefully considering individual histories, presenting issues, and associated risk factors to inform treatment plans.
• Develop and execute comprehensive treatment strategies in collaboration with patients, focusing on promoting emotional well-being, coping mechanisms, and enhancing quality of life during terminal care.
• Deliver evidence-based therapeutic interventions including cognitive-behavioral therapy (CBT), stress management techniques, and grief counseling tailored to individual patient needs.
• Facilitate individual and group therapy sessions that empower patients and family members to express their feelings and develop coping strategies.
• Provide crisis intervention support and develop safety plans during mental health emergencies, ensuring seamless transitions to higher levels of care if necessary.
• Educate patients and families about mental health conditions related to terminal illnesses, treatment options, and community resources available for additional support.
• Maintain comprehensive and accurate documentation of all assessments, treatment plans, and patient interactions in compliance with legal and ethical standards.
• Engage in ongoing professional development and participate in team meetings, contributing to case reviews and treatment strategy development.

Patient Population / Client Focus

You will work primarily with patients facing terminal illnesses and complex psychosocial issues. This role emphasizes compassion and the ability to address not only the mental health dimensions but also the emotional and social challenges of patients and their families during palliative care.

Qualifications

• A Master's degree in Social Work from an accredited institution.
• Valid and current LCSW license in the state of Virginia.
• A minimum of two years of post-licensure experience, preferably within mental health or hospice settings.
• Comprehensive knowledge of mental health disorders, evidence-based interventions, and trauma-informed care.
• Experience in providing therapy and mental health support in high-stress environments.
• Strong communication and interpersonal skills, capable of establishing rapport with patients and families from diverse backgrounds.
• Proficiency in electronic health record (EHR) systems and managing case files electronically.

Preferred Experience

• Experience working within hospice or palliative care environments.
• Familiarity with community resources and support systems specific to patients at the end-of-life stages.
• Demonstrated crisis intervention skills and experience managing acute mental health situations.

Compensation and Benefits

The role offers a competitive salary range of USD 80,000–100,000 annually, commensurate with experience and qualifications.
