Key Responsibilities

• Conduct thorough clinical assessments of patients to understand their emotional, social, and psychological needs, forming personalized treatment plans that adhere to best practices.
• Implement therapeutic interventions such as cognitive-behavioral therapy (CBT), mindfulness-based therapies, and solution-focused approaches in a manner that respects patient's life circumstances.
• Maintain detailed and timely clinical documentation, ensuring compliance with HIPAA and state regulations while protecting patient confidentiality.
• Collaborate effectively with interdisciplinary teams including physicians, nurses, and other social workers to coordinate comprehensive care plans that address the physical and emotional needs of each patient.
• Facilitate family support sessions to educate and guide relatives through grief, loss, and adjustment concerns, providing them essential tools to manage the emotional impact of their loved one’s illness.
• Participate in regular supervision, case consultations, and team meetings to discuss patient progress and enhance care delivery.
• Stay informed on the latest research and evidence-based practices related to hospice care and mental health, applying this knowledge to improve patient outcomes.

Compensation and Benefits

This full-time position offers a competitive salary ranging from USD 80,000 to 100,000 annually, commensurate with experience and qualifications.
