Key Responsibilities

• Conduct comprehensive assessments to identify clients' social, emotional, and environmental challenges, forming the basis for tailored interventions.
• Develop individualized service plans that articulate clear goals, strategic interventions, and quantifiable progress monitoring, ensuring alignment with client priorities.
• Provide compassionate counseling and support to individuals and families navigating mental health challenges, personal crises, and life transitions.
• Engage clients in co-creating personalized service plans, linking them to pertinent community resources and support networks.
• Offer crisis intervention, including real-time risk assessments and safety planning tailored to the client's immediate needs and circumstances.
• Actively collaborate with community agencies and organizations to coordinate multi-faceted services and advocate on behalf of clients for improved outcomes.
• Participate in community outreach initiatives to raise awareness of our agency's services, fostering connections with potential clients and community stakeholders.
• Engage in team meetings, case conferences, and staff training sessions to enhance interprofessional collaboration and maintain high standards of care.

Patient Population / Client Focus

Your primary focus will be on individuals and families facing severe life challenges often exacerbated by socio-economic factors. These may include clients in hospice care requiring emotional support through end-of-life transitions, as well as those grappling with mental health issues linked to poverty, substance dependence, and domestic crises. Your ability to provide trauma-informed care and culturally competent support will be critical in fostering trust and facilitating healing within this sensitive client demographic.

Qualifications

• A Master’s degree in Social Work from an accredited program is required.
• An active state licensure as a Licensed Clinical Social Worker (LCSW) is essential for this role.
• Demonstrated understanding of mental health counseling techniques, as well as social and economic justice issues affecting diverse communities.
• Exceptional communication, cultural competence, and critical thinking skills are mandatory for effectively engaging with clients and colleagues.

Preferred Experience

• Previous experience in a social services agency or a similar setting is preferred, particularly in the areas of hospice care or mental health crisis intervention.
• Familiarity with local community resources and knowledge of trauma-informed care principles are highly beneficial.

Compensation and Benefits

The annual salary for this position ranges from $60,000 to $100,000, commensurate with experience and qualifications.
