Bright Star Counseling is seeking a Licensed Master Social Worker to join our dedicated team in Pittsburgh, Pennsylvania. This full-time remote position is an opportunity for a compassionate and skilled social worker to make a meaningful difference in the lives of seniors and their families. As a part of our hospice center, you will provide essential psychosocial support and care coordination to ensure that our clients maintain a high quality of life.

In this remote role, you will primarily work from home but will need to conduct regular visits at our hospice center and clients' residences. Our work environment is collaborative and supportive, encouraging communication and teamwork among all staff members. You will have the flexibility to manage your schedule while ensuring that client needs are met. Monday through Friday, you will be expected to work from 8:00 AM to 4:30 PM, with the potential for occasional evening or weekend hours based on client requirements.

• Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, social, and mental dimensions.
• Collaborate intimately with clients and their families to devise personalized care plans focusing on their objectives and preferences.
• Provide emotional support and counseling to help clients navigate life transitions associated with aging and health challenges.
• Advocate for clients' rights while ensuring access to vital community resources and services.
• Work in tandem with a multidisciplinary team, including nurses and caregivers, to develop and refine care strategies for our clients.
• Initiate regular home visits to monitor clients’ well-being, assess living conditions, and address any emerging safety concerns.
• Facilitate effective communication among family members involved in the client's care and resolve conflicts gracefully.
• Educate clients and their families about local resources, support groups, and programs designed to bolster social engagement.
• Maintain accurate documentation of clients' assessments, care plans, and progress notes utilizing electronic health record (EHR) systems.
• Stay updated on best practices in social work, related policies, and ethical standards to ensure the highest quality of care.

Our primary focus is on senior clients and their families, navigating the complexities of aging and serious illness. The social worker will play a vital role in ensuring that clients feel supported during this challenging time, serving as a bridge between healthcare services and the emotional and social needs of clients and their families.

• Master's degree in Social Work from an accredited university.
• Current and valid licensure as a Licensed Master Social Worker (LMSW) in Pennsylvania.
• A minimum of two years of experience in social work, particularly within community and geriatric settings.
• Deep understanding of the principles of social work, aging-related issues, and knowledge of available community resources.
• Strong assessment and counseling skills, with an emphasis on empathetic communication and rapport-building.
• Exceptional teamwork and communication skills to effectively engage with diverse multidisciplinary teams.
• Familiarity with electronic health record (EHR) systems and a proficiency in digital documentation practices.

• Experience working directly with hospice clients or in end-of-life care situations.
• Background in conflict resolution and family dynamics specific to senior care.
• Proven track record of advocacy and resource coordination for senior clients.
