Position Overview

Bright Star Counseling is seeking a Licensed Master Social Worker to join our dedicated team in Pittsburgh, Pennsylvania. This full-time remote position is an opportunity for a compassionate and skilled social worker to make a meaningful difference in the lives of seniors and their families. As a part of our hospice center, you will provide essential psychosocial support and care coordination to ensure that our clients maintain a high quality of life.

Work Environment

In this remote role, you will primarily work from home but will need to conduct regular visits at our hospice center and clients' residences. Our work environment is collaborative and supportive, encouraging communication and teamwork among all staff members. You will have the flexibility to manage your schedule while ensuring that client needs are met. Monday through Friday, you will be expected to work from 8:00 AM to 4:30 PM, with the potential for occasional evening or weekend hours based on client requirements.

Key Responsibilities
  • Conduct comprehensive assessments of seniors' psychosocial needs, taking into account emotional, social, and mental dimensions.
  • Collaborate intimately with clients and their families to devise personalized care plans focusing on their objectives and preferences.
  • Provide emotional support and counseling to help clients navigate life transitions associated with aging and health challenges.
  • Advocate for clients' rights while ensuring access to vital community resources and services.
  • Work in tandem with a multidisciplinary team, including nurses and caregivers, to develop and refine care strategies for our clients.
