• Conduct comprehensive assessments of seniors' psychosocial needs, integrating emotional, mental, and social factors into care planning.
• Create individualized care plans in collaboration with seniors and their families, focusing on their specific preferences and goals.
• Provide counseling and emotional support during life transitions, assisting clients with navigating challenges such as loss and health changes.
• Advocate for clients' rights to access necessary community resources, ensuring they benefit from the support services available.
• Coordinate with a multidisciplinary team—comprising nurses, caregivers, and other healthcare professionals—to enhance care delivery.
• Perform regular check-ins via virtual visits to monitor client well-being, assess living conditions, and identify any potential safety issues.

• A Master’s degree in Social Work from an accredited institution.
