• Conduct comprehensive psychosocial assessments to evaluate patients' mental health conditions, strengths, and areas for growth, utilizing tools and methodologies tailored for remote evaluations.
• Deliver evidence-based therapeutic interventions, including but not limited to cognitive-behavioral therapy (CBT) and dialectical behavior therapy (DBT), adapting techniques to a virtual format as necessary.
• Collaborate closely with psychiatrists, nurses, and interdisciplinary team members to create an integrated care plan that addresses the multifaceted needs of each patient.
• Educate patients and their families on mental health diagnoses, treatment modalities, and medication management, ensuring clarity and understanding in all communications.
• Facilitate individual, group, and family therapy sessions, fostering an engaging online environment that encourages participation and connection.
• Develop personalized treatment plans in cooperation with the treatment team, weaving in patient goals, preferences, and evidence-based practices.
• Assist clients in navigating the complexities of the mental health system, serving as an advocate for their rights and needs at every turn.
• Organize and facilitate therapeutic groups and psychoeducation sessions aimed at enhancing patient knowledge and skills for managing mental health challenges.
• Participate in discharge planning meetings to ensure smooth transitions and continued support post-treatment.

In this role, you will work with a diverse population, including children, adolescents, adults, and families facing various mental health challenges. Our patient-centric approach emphasizes individualized care plans that reflect each client's unique situation, encouraging autonomy and resilience throughout the treatment process.

• A Master's degree in Social Work from an accredited institution.
• Active state licensure as a Licensed Clinical Social Worker (LCSW) is required.
• Demonstrated knowledge of a wide range of mental health disorders and treatment methodologies.
• Proficiency in crisis intervention techniques and a strong ability to work collaboratively in a multidisciplinary team environment.
• Excellent communication skills, both written and verbal, to effectively engage with clients and colleagues alike.

• Experience in a community health setting or remote mental health practice is highly desirable.
• Familiarity with electronic health record (EHR) systems to maintain accurate and timely patient documentation.
