Key Responsibilities

As a Licensed Clinical Social Worker at Mind Wealth Wellness Center, your day-to-day responsibilities will include:

  • Conduct comprehensive evaluations and assessments of patients to identify their psychosocial needs and develop effective intervention strategies.
  • Actively participate in Individualized Family Service Plan (IFSP) development, ensuring that each plan aligns with the specific needs and goals of the family.

  • Provide emotional support and counseling to families, helping them navigate the challenges of serious illness and grief.
  • Utilize coaching models and natural environment teaching techniques to facilitate family engagement.
  • Maintain compliance with all applicable federal and state regulations, ensuring the highest standards of care.
  • Develop strong rapport with patients, families, and colleagues, fostering a positive and supportive atmosphere for both caregivers and families.
  • Educate families on strategies to enhance their loved ones’ emotional and psychological well-being.
  • Contribute to a team-oriented approach by collaborating with healthcare providers to align care strategies and enhance overall patient outcomes.
Patient Population / Client Focus

This position primarily serves patients receiving hospice care, along with their families. You will provide critical psychosocial support to individuals facing serious, life-limiting diagnoses and will play a key role in addressing their emotional, spiritual, and practical needs. Your expertise will help families cope with the emotional distress often encountered during this challenging period, ultimately facilitating a more compassionate end-of-life experience.

Qualifications

To be considered for the Licensed Clinical Social Worker position, applicants must meet the following qualifications:

  • A Master's Degree in Social Work or a Doctorate of Social Work from an accredited institution.
  • Be licensed as a Clinical Social Worker (LCSW) by the Department of Public Health.
  • A valid driver's license is required.
