Key Responsibilities

• Conduct comprehensive psychosocial assessments to identify patients' emotional, social, and financial needs.
• Provide individual and group counseling, focusing on adjustment challenges, grief, coping strategies, and emotional support.
• Work collaboratively with medical staff to develop and implement holistic patient care plans, ensuring all aspects of patients’ needs are addressed.
• Coordinate post-discharge services with community organizations, aiming to create continuity of care and enhance patient outcomes.
• Participate in interdisciplinary rounds and treatment planning discussions, sharing insights gained from interactions with patients and their families.
• Maintain accurate and timely documentation in adherence to hospital policies and legal standards.
• Provide crisis intervention when necessary, addressing immediate psychosocial needs to help stabilize patients.

Patient Population / Client Focus

You will primarily engage with patients facing serious illnesses and their families, often at critical junctures in their healthcare journey. This includes providing support for end-of-life issues, grief counseling, and resource coordination, where your input will significantly affect their emotional and practical well-being.

Qualifications

• A Master’s degree in Social Work from an accredited institution.
• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Preferred Experience

• At least two years of experience working within a healthcare or hospital setting, preferably in hospice or palliative care.
• Proficiency in understanding medical terminology, mental health conditions, and crisis intervention techniques.
• Exceptional communication and interpersonal skills, allowing for effective collaboration with patients, families, and interdisciplinary teams.

Compensation and Benefits
