Job description

A key player in our Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) Telehealth program.


The LPN will be responsible for triaging patient needs, managing patient care remotely, and ensuring quality and timely communication between patients and the healthcare team. This is an exciting opportunity to make a direct impact on patients’ health while utilizing innovative technology to deliver care, with a focus on providing services to both English and Spanish-speaking patients.


  • Triage & Clinical Assessment:
    • Conduct remote patient assessments using telehealth technology to determine acuity and clinical needs.
    • Triage patient concerns based on clinical guidelines and protocols, providing initial advice, direction, and escalation when necessary.
    • Monitor patients’ vital signs, health data, and symptoms via RPM devices and electronic health records (EHR).
  • Patient Management & Coordination:
    • Manage and monitor chronic conditions (e.g., diabetes, hypertension, heart disease) by reviewing patient data, assessing trends, and providing follow-up care.
    • Collaborate with physicians, nurse practitioners, and other members of the care team to ensure comprehensive care delivery.
    • Provide education to patients on managing chronic conditions, health maintenance, and lifestyle changes, tailored to their specific health needs.

Bilingual English and Spanish

Current and active LPN license in New York

Minimum of 2 years of clinical nursing experience, preferably in a telehealth, chronic care management, or remote patient monitoring setting
