Key Responsibilities

• Act as a primary liaison between clients, families, and outpatient providers to ensure comprehensive care continuity.
• Collaborate with insurance companies for pre-certification processes and utilization management, ensuring that clients receive deserved services.
• Engage in discharge and aftercare planning, facilitating a seamless transition for clients back into their communities.
• Provide therapeutic meal support, guiding clients to develop healthy relationships with food and nutrition.
• Deliver safe, empathetic, and affirming care tailored to the unique circumstances of each client, considering their intersecting identities.

Patient Population / Client Focus

Our client population is diverse, encompassing individuals with various backgrounds and mental health needs. You will work closely with clients experiencing mental health challenges, ranging from anxiety and depression to trauma and relationship difficulties. Understanding the importance of cultural competence, you will strive to create an inclusive atmosphere that respects and affirms each individual's identity.

Qualifications

• Possess a Master's degree in Social Work from an accredited institution, demonstrating a strong foundation in clinical practice.
• Hold a valid Licensed Clinical Social Worker (LCSW) license in the state of Texas, indicating expertise and competency in the field.
• Have experience working effectively with older adults, showcasing your adaptability and versatility in providing age-appropriate care.
• Exhibit strong clinical and diagnostic skills that enable you to assess clients' needs accurately and develop effective treatment plans.
• Demonstrate excellent communication and interpersonal skills, facilitating strong therapeutic alliances with clients and collaboration with multidisciplinary teams.
• Be able to work independently while also thriving in a team-oriented environment.

Preferred Experience

• Experience in a community health clinic setting is a plus.
• Familiarity with various treatment modalities, including Cognitive Behavioral Therapy (CBT) or Dialectical Behavior Therapy (DBT).
• Previous experience with case management and crisis intervention strategies.

Compensation and Benefits
