• Conduct thorough assessments and, when necessary, diagnoses of patients to evaluate their mental health status, employing standardized diagnostic tools like the Diagnostic and Statistical Manual of Mental Disorders (DSM).
• Develop individualized care plans that respect cultural diversity and accommodate the specific needs of each patient, integrating evidence-based practices to optimize mental health outcomes.
• Continuously monitor and assess patient progress regarding their therapeutic goals, adjusting strategies and interventions as needed to ensure effective mental health treatment.
• Maintain accurate and up-to-date treatment records, utilizing electronic health records (EHR) systems and ensuring compliance with regulatory standards and internal protocols.
• Collaborate with patients, families, and interdisciplinary teams to design comprehensive treatment strategies, recognizing the importance of incorporating family dynamics and social systems into care.
• Establish and nurture relationships with referral sources and external providers, facilitating seamless transitions and continuity of care for our patients.
• Participate in mentorship and educational opportunities for students and interns, contributing to the professional development of the next generation of mental health professionals.
• Engage in peer reviews and clinical collaboration, offering insights and support to enhance team member competencies.

• A Master’s degree in Social Work from an accredited institution.
• Current and valid State License as a Licensed Clinical Social Worker (LCSW).

• Experience in outpatient mental health settings and familiarity with working alongside interdisciplinary teams in a healthcare environment.
• Prior experience with patient populations impacted by chronic illness, particularly cancer, is highly regarded.
