• Conduct comprehensive psychosocial assessments through structured interviews, detailed observations, and thorough reviews of medical records to identify and evaluate emotional, social, and environmental factors affecting patient recovery.
• Provide individualized counseling sessions for patients and their families, focusing on adjustment issues related to medical conditions, emotional health, and coping mechanisms to foster resilience and well-being.
• Collaborate with a range of rehabilitation providers to craft and implement tailored treatment plans that address each patient's unique psychosocial needs within the scope of their physical rehabilitation.
• Assist patients in navigating community resources, leveraging connections with support groups, vocational rehabilitation services, and housing assistance programs that facilitate their return to independence.
• Educate patients and families about available community resources and assist in post-rehabilitation planning, ensuring a seamless transition from our care back into everyday life.
• Engage in proactive discharge planning, ensuring that patients and their caregivers are well-informed and supported during the transition back to home or other care settings.

Our patient population encompasses individuals recovering from a variety of conditions, such as strokes, spinal cord injuries, orthopedic surgeries, and neurological disorders. As an LCSW, you will play a critical role in addressing the emotional and psychological aspects of recovery, ensuring that each patient's intrinsic needs are met as they progress through their rehabilitation journey.

• A Master’s degree in Social Work from an accredited institution is required.
• An active state license as a Licensed Clinical Social Worker (LCSW) is a must.
• The candidate should demonstrate a strong understanding of psychosocial dynamics, including adjustment issues, grief, and effective coping strategies.
• Effective communication and interpersonal skills are essential for fostering meaningful relationships with patients and their families.

The annual salary for this role ranges from $75,000 to $100,000, depending on experience, plus a generous sign-on bonus.
