Remote Hospital Pharmacist Overview PharMerica is hiring a Remote Hospital Pharmacist to support our long‐term care and senior living pharmacy operations across Texas. The role involves real‐time medication order verification, clinical review, and regulatory compliance for critical access hospitals and behavioral health settings. Key Facts Schedule: 7 days on / 7 days off (evenings or nights preferred) Location: Fully Remote Setting: Critical Access Hospitals (CAH) + Behavioral Health Licensure Required: Active Pharmacist License in Texas and New Mexico, willingness to obtain additional state licenses Experience Preferred: Inpatient hospital pharmacy or remote verification/clinical pharmacist role Responsibilities Review physician medication orders for appropriateness, dose, route, and potential interactions.

Provide clinical recommendations to physicians and staff regarding therapy changes and optimizations. Maintain up‐to‐date patient/resident profiles, including medical history, lab results, and current medications. Ensure lab tests for high‐risk medications are ordered, reviewed, and communicated.

Collaborate with physicians, nurses, and other professionals for clinical and technical support. Document all communications and interventions in the patient's medical record. Implement and enforce policies for safe handling, dispensing, and oversight of medications.

Ensure compliance with state and federal regulatory requirements. Report medication‐related errors and adverse events to the Pharmacy Manager. Educate clinical teams on safe medication handling procedures.

Perform additional pharmacy tasks as assigned. Qualifications Bachelor's Degree in Pharmacy (Pharm D preferred). One year of experience in long‐term care desired.

Current and valid pharmacist license in the state of practice. Knowledge of drug therapy guidelines, medication safety practices, and regulatory standards (CMS, DEA, FDA). Proficiency with electronic medical records and pharmacy information systems.
