• Engage with clients and establish therapeutic relationships, conducting initial assessments and ongoing evaluations to develop personalized treatment plans.
• Act as a liaison between clients, their families, and outpatient service providers to coordinate comprehensive care.
• Communicate effectively with insurance companies for pre-certifications and manage utilization review processes to ensure patients receive necessary services without interruption.
• Participate in planning for client discharge and aftercare, ensuring a smooth transition and continuity of care.
• Facilitate therapeutic meal support, providing guidance that fosters a healthy relationship with food and addressing underlying emotional issues related to eating.
• Deliver empathetic, affirming care based on an understanding of the diverse and intersecting identities of clients, ensuring a personalized and respectful approach to each individual’s circumstances.
• Document case notes and maintain accurate records in compliance with organizational standards and state regulations.

As a LICSW at Wellmore Health Center, you will work with a varied patient population that includes adults seeking support for mental health challenges, including anxiety, depression, trauma, and more.
